It was going to be explained in a short story of mine that will be posted soon, but to explain, His body is made out of a sort of gooey black mass. (That's the trail he leaves behind) the trail is made out of thin wooden panels- the frail state he's in at the moment. He repeats his living nightmare: getting either ignored completely or insulted by his appearance. That twisted trail of wooden surfaces in a endless black void is the representation of his life, or how he sees the world. A black void of nothingness pressuring him, as the anxiety grows and he is left in pain and loneliness.
(there's a story of how he even came to be, but that's too long to explain in a comment.)

This is eerily similar to certain dreams I have. I also found the wind up key to be significant, as it implies he feels out of control, or forced to continue when he would rather not. You did a good job of making him look like he's walking in a mechanical fashion too. When you said twisted wooden surfaces, I was reminded of the first room in Woodfall Temple in Majora's Mask, which also has kind of a black background, and features a sad wooden creature which Tatl comments on as looking sad or stuck.

You're correct about the wind up key!
it is one of the reasons why he turned like that gooey state.
havent played majora's mask yet (its part of the legend of Zelda series I believe?), but I should look it up.
